Health Screenings
We require state mandated physicals upon entry into kindergarten or grade 1, grade 6, and grade 11. The child’s family physician should perform these exams to provide continuity of care. Forms are available in the school nurse’s office and under the resources area of this page.
We also require state mandated dental exams upon original entry into kindergarten or grade 1, grade 3, and grade 7. The child’s family dentist should perform these exams to provide continuity of care. We urge families who do not have a dentist to contact the school nurse for assistance.
